We provide a universal health insurance claim form (HCFA) to you at each visit. This form can then be submitted to your insurance.
Payment is due at the time services are rendered. We accept cash, debit, and credit cards including Health Savings Accounts (HSA). If you have submitted an insurance claim that is accepted and a check is sent to us, we will credit your account for future visits or send you a reimbursement check.
